INSTRUCTIONS: Use these questions as a guide and note below any concerns or ways GBC can minister to the family or individual. Provide on-the-spot guidance and loving care as needed. Provide direction to resources within the church ministry as needed. Follow-up with family as needed if you make any commitments to help or connect them with a ministry, such as counseling ministry or mentoring ministry. You DO NOT have to read these questions verbatim. They are just a guide. Nor do you have to take notes on every answer. Just action items. Use this as warm relationship-building time.
1. Individual Walk with Christ
- How would you characterize your walk with Christ on a scale of 1 – 10? (1 is spiritual dry and constantly discouraged and 10 is not perfect, but vital and loving the Lord and His Word)
- If below 5, what could we do to encourage you?
2. Marriage
- If married: How would you characterize the growth of your marriage? What do you (each individual) need to do to be more Christ-like as a husband or wife?
3. Children
- If children: Have your children professed faith in Christ and/or which of your children are you still watching for fruit of salvation? [This is a great chance to share the gospel with the children].
- How would you characterize your consistency of lovingly disciplining children? Any special challenges in this?
4. Family
- How would you characterize your overall family life? Enough time together? Growth in relationships?
- Are you having any major struggles we can help with? (Finances, illness, other challenges).
5. Body Life at GBC
- How often would you say you attend Sunday morning service? i. ...Sunday School? ii. ...Sunday evening service?
- Are you involved in any significant relationship-building opportunities, such as small groups, mentoring, etc.?
- Are you currently serving at GBC and do you feel you could do more or less?
- If more, what areas of ministry could you help in?
- Are you able to give faithfully and consistently to GBC? (No $ amount needed)
- Have you taken or plan to take advantage of major discipleship opportunities such as Bible Training Institute or Fundamentals of the Faith?
- Are there any GBC members or attendees with whom you have a current unresolved disagreement?
- Are there any issues that you feel may prevent you from partaking in the Lord’s Table?
